I made an animated, decapitated head monster for an encounter before. As I often describe battles areas / known for heavy monster traffic as having discarded limbs & bones scattered around bushes or up a tree so the head ambushed them from behind it freaked the hell out of everyone and the Wizard was completely caught off guard in the back of the group.
